Preventive Care at Home on Boston's North Shore

Most serious health problems don’t arrive overnight. They build slowly: a blood pressure that creeps up, a blood sugar number that’s a little high, a habit that has been wearing on your body for years. Preventive care is about noticing those things while you still have the most options.

I’m Kate, a family nurse practitioner, and I provide preventive care at home across the North Shore. Together we look at your history, your risks, and your daily routine, and build a plan to help you stay well.

Why Waiting for Symptoms Doesn't Work

By the time something hurts, it has often been developing for a while. High blood pressure, early diabetes, and cholesterol problems frequently cause no symptoms at all. Without someone looking, they can go unnoticed for years.

That doesn’t mean you should worry all the time. It means a little attention now can make a real difference later, and it’s much easier to change a habit than to treat a complication.

What Preventive Care Looks Like With Me

I begin by learning your story: your health history, what runs in your family, what you’ve been told in the past, and what concerns you. Then I check your vital signs and look at the numbers that matter for your situation, like blood pressure and weight.

Next, we talk about your risks in plain terms. I review which screenings and vaccines may be due and explain why each one matters. We look at your diet, activity, sleep, and routines, and pick a few changes that are realistic for you.

I don’t hand you a long list of rules. We choose a small number of steps, and I check in on how they’re going. You’ll also know what to watch for between visits.

What Each Visit Includes

Who This Is For

This kind of care tends to help most when

Heart disease, diabetes, or another condition runs in your family

You were told your numbers are “borderline” or “a little high”

You’d rather avoid problems than wait for them

You haven’t kept up with screenings and vaccines

Working With Your Doctors

I’m not here to replace your primary care provider. I work alongside them. After a preventive care visit, I pass along what we found and what we discussed, so your records and your plan stay in step. If a screening or test needs to be arranged, I help you work out who to ask and what to say.

That way you aren’t trying to remember it all on your own, and your doctors see the full picture.

Preventive Care or a Wellness Visit?

Preventive care focuses on lowering specific risks, like those from family history or borderline numbers. If you’d rather start with a general check-in on how you’re doing, Wellness Visits are the closer fit. If a condition has already been diagnosed, Chronic Disease Management gives you regular follow-up, and Medication Management helps with the pills that come with it.

Helping a Parent Stay Ahead of Problems

If you think about a parent’s health from a distance, preventive care gives you something concrete to do. I can visit them at home, look at their risks and what’s due, and talk through a plan they can follow. With their permission, you can join by secure video, hear it yourself, and ask your own questions.

Cost and Scheduling

My services are private duty and self-pay. Please call or email for current rates before your first visit. I see patients Monday to Friday from 8 to 6 and Saturday from 10 to 3, at home or by secure video.
