Living with diabetes, heart failure, or high blood pressure requires daily care. I’m Kate, a Family Nurse Practitioner providing at-home chronic disease management across the North Shore—so you’re always supported between doctor visits.

My visits are unhurried. I start by asking how you’ve been feeling, because that tells me more than any chart. Then I check your vitals, look at the readings you’ve been keeping, and go through your medications with you.
Being in your home matters. I can see where your pills are kept, whether your blood pressure cuff or scale is easy to reach, and what your day actually looks like. A clinic visit can’t show me those things, and they often explain why a condition isn’t under control.
After each visit you know where things stand, what to do before I see you next, and which symptoms mean you should call.
I work with people living with

Diabetes

Heart failure

High blood pressure (hypertension)

Other ongoing conditions that need regular follow-up
If your condition isn’t on this list, call me at 617-299-9590 and ask. I’d rather talk it through than have you assume I can’t help.

Your condition never seems fully under control, even though you’re doing what you were told
You were recently diagnosed and are still trying to make sense of it
You’re managing more than one condition and several medications
Getting to appointments is hard, tiring, or just takes too much out of the day
I’m not here to replace your primary care provider or your specialists. I work alongside them. After I see you, I pass on what I found, including changes in your readings, new symptoms, and questions you didn’t get to ask. If something needs to change in your treatment, I talk with your doctor directly.
That means you stop being the messenger between offices. If you see a cardiologist as well, I can help keep that connection going through Coordination with Specialists. And because medications often cause the most trouble, many patients also use Medication Management.

Not every check-in needs me in your living room. Secure video visits are available for quick follow-ups, for going over your readings, or for including family members who live far away. Your daughter in another state can join the call and hear the plan for herself.
It's ongoing care for a long-term health condition. It covers regular check-ins, tracking your symptoms and numbers, reviewing your medications, and keeping your doctors informed, so small changes get noticed before they turn into emergencies.
I work with diabetes, heart failure, high blood pressure, and other ongoing conditions. If you're unsure whether yours qualifies, call 617-299-9590.
That depends on you. After the first visit we decide together what makes sense. Some people need a visit every few weeks, and others need less often, with video check-ins in between.
No. I work with your primary care provider and specialists, and I keep them updated.

Call 911 for chest pain, trouble breathing, signs of a stroke such as sudden weakness, face drooping, or slurred speech, or severe confusion. Home visits are for ongoing care, not emergencies.

Navigating the healthcare system can often feel confusing and overwhelming, especially when facing a new diagnosis, managing chronic conditions, or recovering from a medical procedure.
