Wellness Visits at Home on Boston's North Shore

Not every visit has to be about something going wrong. Some of the most useful ones happen when you feel fine and someone takes the time to look at how you’re really doing. A regular wellness visit is exactly that: a calm, unhurried check-in, before small changes turn into bigger problems.

I’m Kate, a family nurse practitioner, and I provide wellness visits at home across the North Shore. I come to you, so there’s no drive, no waiting room, and no rushing through your questions.

Why a Check-In at Home Is Different

In a clinic, you get a short window and a lot to cover. At home, I see how you actually live: how you move around your rooms, whether you’re eating well, how you handle your medications, how the stairs are going. Those details rarely come up in a ten-minute appointment, and they often tell me the most. It also helps people who find appointments tiring or stressful. When you’re comfortable, you tend to talk more openly, and the visit is more useful.

What a Wellness Visit Looks Like With Me

I begin with a conversation. How have you been sleeping? Eating? Getting around? Is anything bothering you that you haven’t mentioned to anyone? Then I check your vital signs and go through your medical history, so I can see how your health is trending over time.

I look at your medications, ask about any recent changes, and notice the small things: a new unsteadiness, a change in appetite, or a shift in your numbers.

We finish with practical advice for everyday life: nutrition, sleep, staying active, and what to watch for. You’ll know where you stand and what, if anything, to do next.

What Each Visit Includes

Who This Is For

This kind of care tends to help most when

You feel fine, but haven’t had a real check-up in a long time

Getting to the doctor’s office is tiring or hard
You want someone to keep an eye on your health over time

You’re a son or daughter who worries about a parent living alone

Working With Your Primary Care Provider

I’m not here to replace your primary care provider. I work alongside them. After a wellness visit, I pass along anything they should know, such as changes in your readings, new symptoms, or concerns you raised. If something needs a closer look, I tell you plainly and help you take the next step.

That way your regular care and my visits fit together, and nothing you mention goes unheard.

Wellness Visit or Preventive Care?

A wellness visit is a regular check-in on how you’re doing right now. If you want to focus on lowering a specific risk, such as a family history of heart disease or diabetes, Preventive Care is the closer fit. If you live with a long-term condition, Chronic Disease Management gives you more regular follow-up, and Medication Management helps if your pill list has grown.

Helping a Parent Who Lives Alone

If your mom or dad lives on their own and you’re not nearby, a regular wellness visit lets someone you can trust see how they’re really doing. I check their health, notice changes, and talk through what I find. With their permission, you can join by secure video, hear it yourself, and ask your own questions.

Cost and Scheduling

My services are private duty and self-pay. Please call or email for current rates before your first visit. I see patients Monday to Friday from 8 to 6 and Saturday from 10 to 3, at home or by secure video.
