If you see a cardiologist, an endocrinologist, a kidney doctor, and your regular primary care provider, you’d think they were all talking to each other. In practice, they often aren’t. Notes don’t get shared, a test result sits in one office, and a medication change one doctor made never reaches the others. Too often the patient ends up as the messenger.
I’m Kate, a family nurse practitioner, and I provide specialist care coordination at home across the North Shore. With your permission, I stay in touch with your doctors, so your care fits together instead of being scattered across different offices.

It’s rarely one big mistake. It’s a referral that never gets scheduled, a result nobody mentions, or a visit where you realize the specialist hasn’t seen your latest labs. Each one is small. Together they can leave you confused, repeating your story, or holding two sets of instructions that don’t agree.
Appointments are short, and it’s hard to know what to ask. You might leave a visit unsure what was decided or what happens next. You shouldn’t have to piece it together yourself.

I start by getting the full picture: who is on your care team, what each doctor is treating you for, and what each one has told you to do. I ask about your referrals, your upcoming appointments, and any tests you’re still waiting on.
Then, with your permission, I get in touch with your doctors’ offices. I share what I’ve seen at home, ask about results or plans that haven’t reached you, and look for instructions that don’t agree with each other, such as overlapping medications or conflicting advice.
After that, I explain it all to you in plain words, so you know what each doctor is doing and why. You leave with a clear picture of what happens next.

I’m not here to take over from your specialists or your primary care provider. They know their areas well. My job is to connect them. After each visit I pass along what I found at home, including symptoms, readings, and the questions you didn’t get to ask.
If something needs a decision, I contact the right doctor directly, so you aren’t stuck in the middle between offices.

Coordination is about keeping your doctors connected. If you also want someone beside you at appointments, Patient Advocacy covers that. Medications often cause the most conflict between doctors, so Medication Management is a natural pairing, and Chronic Disease Management helps if you’re living with a long-term condition.

It's making sure your doctors are working from the same information. I keep track of your referrals, results, and treatment plans, and I stay in touch with your care team so nothing gets lost between offices.

Navigating the healthcare system can often feel confusing and overwhelming, especially when facing a new diagnosis, managing chronic conditions, or recovering from a medical procedure.
